Systemic lupus erythematosus (SLE) is a chronic autoimmune disease that affectsvarious organs and systems of the body. Its pathophysiology is based on the production of autoantibodies and the occurrence of inflammatory reactions, leading to damage to tissues, such as skin, joints and kidneys. Despite significant advances in research, the mechanisms that explain the onset and progression of the disease are still not fully delinatedExtracellular vesicles (EVs) are small membrane-bound structures that are secreted by cells and have a major role in intercellular communication by transporting proteins, lipids and nucleic acids. Recently, there has been increasing interest in EVs as they appear to be involved in immune processes and the pathogenesis of autoimmune diseases. This paper examines the contribution of extracellular vesicles to the pathogenesis of SLE, explaining how they influence the immune response and enhance inflammation. At the same time, their usefulness as diagnostic biomarkers and as target for new therapeutic approaches is explored.
